Summary:

Crossroads is requesting approval of the attached Independent Service Agreement with Laurie Gulutzan, a licensed MFT contractor, to provide individual counseling, couples counseling, family sessions and case management to their students. Ms. Gulutzan will be employed 30 hours per week. Approximately 25 hours per week will include counseling, case management, administrative and other services provided at the school. The remainder of the time will include consultation.
